IMPORTANT: Starting March 5, 2022, OmniBay will charge a $99 (one-time*) admin/processing fee to process your contracting forms. This fee covers the costs associated with processing your contracting forms, including but not limited to: platform fees and admin fees. This fee applies to all newly contracting team members - regardless of which contracting option they choose.
*This fee is charged one-time per contracting process. For example, if you choose Option 1, then want to switch to Option 2 later, you will be charged this fee twice).

Contracting Instructions From the Video
Contracting on our team has two major benefits! The first benefit is that you will get access to our core network of insurance carriers at commission levels up to 60% higher than other agencies AND you will also get access to our leads platform and discount club at no additional cost.
In this video, we will walk you through the contracting process so that your documents are approved the first time around.
To get contracted you will need to submit, pages 2,3,4 & 5 of the ACT Appointment Data Sheet (which can be found below).
You will also need to submit:
- a voided check or direct deposit form,
- An Errors & Omissions Certificate of Liability showing your name as the insured and a minimum of $1 million in coverage. If you don’t have sufficient errors and commissions insurance, please use the link on this page to purchase it now.
- Your resident state life and/or life and health insurance license, and;
- the front and back of your driver’s license or state ID card.
To ensure that your documents can be processed in a timely manner without being rejected or delayed, please listen close to all of the following requirements. Failure to complete any of them will result in your forms being rejected.
- All documents must come in the same submission - as we do not accept piecemeal submissions.
- When completing the ACT form it must be signed by hand and be completed in its entirety. It is recommended that you type or neatly print your responses on this form using black ink. Illegible submissions will not be processed.
- The voided check or direct deposit form must have your name printed on it.
- Your resident state insurance license must be valid, meaning not expired and must show Life and/or life and health as a line of authority.
- The Errors & Omissions Certificate of Liability must show your name as the insured and have a minimum of $1 million in coverage.
- If you would like to contract as an insurance agency, your Errors & Omissions Certificate of Liability, insurance license, and your voided check MUST have your agency name printed on each of the three.
- Additionally, if you plan to do business as a company, both your name and the name of your company are required on pages 2 & 4 of the ACT Appointment Data Sheet.
- Finally, it is ok to submit two separate state licenses, one with your name listed and one with your company’s name listed.
